The best (and of course most expensive) hotel is the Hotel Grand Europe, right on Nevskiy Prospect.  An easy walk to many attractions (Church of Spilled Blood, Hermitage, Kazan Cathedral, Russian Museum, Mussorgsky Theatre).  It has three main restaurants.  We did not eat in their "cuisine" restaurant, but did eat in their Italian restaurnt twice, once for lunch and once for dinner.

We stayed at the Astoria (also a 5 star hotel, slightly cheaper than the Grand Europe), right across from St. Isaacs (great views from the dining room) and were generally pleased with it.  They arranged our private tours to the country palaces, got us theatre tickets and cars, and helped us arrange a special birthday celebration (all for a price, of course).  The only qualifcation is the main restaurant.  We ate there our arrival night.  It was expensive and not good.  Oddly enough, the food at the bar was excellent.  Don't miss the caviar and blinis!

For a nice 4 star hotel (they call it a 5 star on the web site, but it looked a step down from the Astoria), try the Astoria's sister hotel next door, the Angleterre.  We ate there several times for lunch and once for dinner.
